Is another component, after fulfilling the three, to ask/pray Ha-Shem (God) to take away the burden of guilt/shame/remorse etc. and then surrendering that to Ha-Shem - letting it go? Additionally, is there at the end asking Ha-Shem to bless us with a better life? As you stated, a change of heart and of person is the sign that the Teshuva is effective/successful/approved.
